By ticking the consent box when booking, I confirm the following. This consent applies to my first session and to all subsequent sessions, packages and programs with Pure Vitality Coaching FZCO, including sessions that Pure Vitality Coaching FZCO schedules on my behalf at my request. It remains valid until I withdraw it in writing (team@pvcoachings.com) or until I am asked to accept an updated version.

Voluntary participation: I take part voluntarily in life coaching, personal training and/or lifestyle coaching provided by Pure Vitality Coaching FZCO, online or in person.

Coaching, not therapy: I understand that coaching (mental, emotional and fitness) is not a substitute for medical or psychological treatment and does not include diagnosis, therapy or treatment.

My health: I confirm that I am in suitable physical and mental health to participate. I have consulted a qualified healthcare professional if I have any medical or psychological condition (including heart conditions or mental health disorders). For personal training, I will complete the health questionnaire truthfully and completely before my first session and inform my trainer immediately about any change in my health, including new injuries, pain, pregnancy, medication or symptoms.
Before every session, I make sure that the statements above are still accurate and inform my coach in advance of any change.

Risks: I understand that exercise involves inherent risks, including muscle soreness, strains, sprains, falls and, in rare cases, serious injury or cardiovascular events. I understand that personal development work can involve emotional discomfort. I accept these inherent risks.

During training: I follow my trainer's instructions and train within my limits. I stop immediately and tell my trainer if I feel pain, dizziness, chest discomfort or shortness of breath. For sessions at my home, in my building gym or online, I am responsible for a safe training space and for the condition of my own equipment.

Emergencies: I understand that coaching is not an emergency service. In an emergency, I will contact my local emergency services.

Release: To the extent permitted by UAE law, I release Pure Vitality Coaching FZCO and its coaches from liability for injuries or losses that result from the inherent risks of the activities described above. This does not apply to liability that cannot be excluded under UAE law, including liability for harm caused by negligence, gross negligence or wilful misconduct.

My data: I explicitly consent to Pure Vitality Coaching FZCO processing the health and personal information I share, so that it can deliver its services safely, as described in the Privacy Policy. I can withdraw this consent at any time.
